The OS for longevity medicine — for patients who feel well and stay with you for decades
iksa is the harness for n‑of‑1 medicine. You can run decades of prevention like a trial’s follow-up, with every ApoB held to the target you wrote and your nurse on whatever moved.

The plan for one heart comes from reading a scan, a blood draw, a genome and a treadmill together
Your patients feel well and you still follow them the way an oncologist would, scan against scan and ApoB against a target that can move, each of them a trial of one. The population tells you where a patient started, and their last reading tells you what changed.
- Coronary CTThe calcium score by age, sex and race, and the non-calcified plaque a calcium score can’t see, so the lipid plan answers what is in this artery. A first scan by Year 2, then an interval you set from its plaque, since no trial has tested repeat scanning in people without symptoms.
- ApoB and Lp(a)One Lp(a) result, mostly inherited, tells you how low ApoB has to go, because ApoB counts each Lp(a) particle once, and a high Lp(a) adds risk of its own. ApoB within three months of any change, then every three to six months. Lp(a) once in every adult, as the 2026 ACC/AHA guideline recommends, a risk enhancer at 125 nmol/L or more.
- The genomeLDLR, APOB and PCSK9 can name familial hypercholesterolemia. A polygenic score, read against the ancestry it was built on, is now a risk enhancer in the 2026 ACC/AHA guideline. LPA says a high Lp(a) is inherited, while the measured level, once, sets the plan, and APOE speaks to the brain as well as the lipids. Sequenced once, at baseline, and re-read as the evidence moves.
- Blood pressure, VO2 max and sleepHome blood pressure, VO2 max for age and sex, and resting heart rate, HRV and sleep beside the patient’s baseline, so you change the training block on evidence, whatever the week felt like. A VO2 test once or twice a year. Sleep every night.
- mTOR inhibition and senolyticsA weekly mTOR inhibitor missed its primary endpoint, visceral fat, over 48 weeks in PEARL, with lean mass and pain better only in women on the higher dose. A senolytic combination missed its primary bone endpoint in 60 postmenopausal women before meeting its primary fibrosis endpoint against placebo in 31 adults with MASH (Nature Metabolism, October 2026). Efficacy against aging is still to be proven for both, and what a program reads today is each patient’s safety labs and function. An mTOR inhibitor weekly, with safety labs every four weeks in the nearest trial; the senolytic in pulses, the kinase inhibitor for two days and the flavonoid for three, every 28 days, five times over 20 weeks in the largest trial.
- Plasma exchange and hyperbaric oxygenTherapeutic plasma exchange lowered 15 epigenetic clocks against sham in 42 healthy adults over 50, with no functional outcome measured, and slowed functional decline against sham in moderate Alzheimer’s, not mild. Full-pressure hyperbaric oxygen’s gains, from unblinded single-center trials, don’t carry to a mild chamber. Exchange twice in one week each month, best with IVIG, or once a month in the aging trial, weekly for six weeks then monthly for a year in Alzheimer’s, and sixty full-pressure sessions in twelve weeks.
The patient met their ApoB target, and the doctor moved the target
The patient’s calcium score of 38 read as mild until their age, the plaque on their CT and the Lp(a) they inherited changed what it meant.

Does your protocol say what to do when a panel reads in range and the scan or genome doesn’t? That’s the part of it we’d most like to walk through with you.
Request a walkthroughRequest a walkthrough for longevityNothing brings a patient who feels well back to you, so the follow-up a trial gives a whole team is yours
A pharma trial runs on trial management, data capture, safety reporting, biostatistics and a team of people around them, and no clinic can afford that for one patient, so with no payer in the room the clinical, the commercial and the regulatory record all fall on you.
Trial management
Which scan is due, which re-test you dated and what you were watching for, across all your patients and years, held in your head and your senior nurse’s.
Data capture
A CT report, a genome PDF, a stack of lab PDFs and a wearable, opened one at a time and copied out by hand the evening before each follow-up.
Safety reporting
A side effect after the patient’s lipid lowering stepped up reaches your nurse as a text, and a trial would give that text a coded term, a severity grade and a monitor.
Biostatistics
Whether a new ApoB is the plan, the lab or the season is a statistician’s question in a trial, and here you read it with each earlier panel open in its own file.
Once the doctor writes ApoB under 60, the nurse’s list carries the patient’s ApoB and the new target gets explained when they ask
The harness is the system of action around that decision: it watches the markers the doctor follows and what the patient reports, and puts agents to work proposing the next step, up to where the nurse or the doctor decides.

Before the review, the doctor asks the patient’s whole record what changed
Every answer cites the note, the chart or the report it came from, in the record’s own words, and when the record holds nothing, it says so.
Clinical Intelligence: AI for longevity medicineThe nurse works from the labs the doctor is watching, with that ApoB and its status on the list
Your nurse reviews and resolves what’s flagged. When the next person writes in with a CT in hand, Agentic CRM drafts from your approved answers, says where they stop and hands anything medical to a person, and your team approves or edits the draft before anyone sends it.
Agentic CRM: scheduling and follow-upThe patient can ask what an ApoB under 60 means for them and hear why the doctor set it there
Patient AI knows their protocol and history and explains what the doctor wrote. A new symptom gets safety questions first and, when it needs a person, reaches the nurse or doctor; a question about stepping up their therapy is the doctor’s call and, with the patient’s yes, goes to the nurse.
Patient AI: the treatment companion
Every year of one patient’s care leaves evidence behind, and across your patients that is where a paper on your own protocol starts
Every reading keeps its date and source report and each plan item its note, so the patient’s years read as evidence for you in Year 12. Across your patients, de‑identified and with consent, those years are where a first study with a research group would start. How a first study starts from care →

What a longevity physician asks first
Can I keep my own panel, my own targets and my own cadence?
All three.iksa reads your labs and your genetics reports from supported vendors on the cadence you set, with each patient’s years behind it, and every decision stays written in your note.
Do the CT and the genome report sit with the labs?
On one record.A coronary CT, a whole-body MRI or a genome report is filed as the document it is, dated, beside that year’s labs. iksa reads what your note says about a scan, and a supported vendor’s genome report itself, re-finding each gene, rsID and genotype before it counts.
Who holds a patient’s genome?
We do, for your clinic. The BAA is signed with you before any patient data moves, and their genome report sits under it, never sold or used for marketing or advertising. Access is set by role, and every action on their record leaves an audit trail your clinic can read.
While you’re with someone else, a patient asks about their numbers. What do they get?
What you wrote.Patient AI explains the result from their own record and your plan, with their numbers beside it.
If a reading gets flagged, who changes the plan?
You do.iksa surfaces, flags and drafts, and a clinician makes every clinical decision. A change to the protocol is written in your note, and what iksa reads next comes from that note.
Do I have to leave the systems my team already runs on?
They stay.Your EHR stays the record of each visit, and the calendar stays where it is. iksa reads your charts from your EHR, by connection or by upload, and holds what runs across those visits: the protocol, every report and every decision.
Vendors keep pitching me longevity clinic software as an OS for longevity medicine. Is this one?
Closer to a harness. We call it the harness for n‑of‑1 medicine, because a longevity plan gets rewritten whenever a scan, a genome re-read or the next ApoB says more, and what runs around that plan has to move when you rewrite it.
